At Impact Life, we offer a wide range of service options across Virginia and customize support based on each individual’s needs. Our goal is to provide families and coordinators with meaningful choices and person-centered solutions.

Our admissions team is here to guide you through the process and ensure that every referral is handled with care, respect, and professionalism. Please contact us for information about current openings, as new homes and service options are continually being developed.

To begin the admissions process, we will need the following documentation from the individual’s case manager or support coordinator:

  • Psychological Evaluation

  • Current Physical & List of Medications

  • Supports Intensity Scale (SIS)

  • Risk Assessment

  • VIDES

  • Person-Centered Plan

Our team is available day and night to assist with urgent needs or problem-solving. While emergency services are always the first point of contact in medical crises, our staff is ready to support providers and families whenever needed.
